The Community Supports Network, Inc. (CSN) supports persons, both children and adults, with mental
health needs and/or intellectual disabilities gain skills, knowledge and experience to increasingly use and
benefit from the resources and settings available to all citizens in our community.
Position Purpose
To conduct referral reviews, complete intake activities and provide on-going monitoring of medical
supports provided to customers to ensure fulfillment of CSN Mission and achievement of Organizational
Outcomes. To deliver high quality services that support customers to learn, grow and live their best
possible lives and thrive as members of their community

Job Duties
1. Lead effective intakes by coordinating meetings and information sharing with customers,
stakeholders and employees regarding customer eligibility, service authorizations and options,
service needs and preferences. Obtains all necessary consents, completes a social history and other
agency-required forms/paperwork, records and documentation.

2. Promote health and wellness of persons supported by
  • Organizing, overseeing and assisting when necessary in the delivery of personal care,
    medical appointments, medication administration, and assisting persons receiving services to
    take an active role in their health care decisions.
  • Facilitates the completion of required assessments, services and safety plans, residential
    agreements, and coordinates with the individual’s team to develop service, medical and
    behavioral support protocols.
  • Arranges appointments (e.g., physician, vision, dental, etc.) and provides transportation and medical
    supports (e.g., medications, medical equipment, etc.), and ensures the agency team is
    trained and ready to provide the necessary services and supports to meet the individual’s
    needs and preferences.
  • Monitors to ensure that significant changes in the individual’s needs are addressed immediately, that the service plan is revised, and that gaps in services are addressed by the agency team.

3. Enhance the ability of the person supported to lead a self-determining life by providing the
support and information necessary to build self-esteem, be assertive, make decisions and gain
access to their community
4. Be a strong advocate for the person supported by being knowledgeable about the diverse
challenges facing persons supported, identify and use effective advocacy strategies to support the
person to overcome challenges they face.

5. Recognize the importance of friends, family, and community relationships and deliver specific
supports and interventions to meet the unique needs of the person supported.

6. Establish a collaborative relationship with the person supported and those who support him
or her through positive communications and cooperative interactions.

7. Preserve the dignity of persons supported by supporting them to evaluate and determine the
effectiveness, appropriateness and quality of his/her services/supports.

8. Provide Person Centered Supports by facilitating what is important to the person, which requires
continual listening and learning in alliance with the person, their family and friends.

9. Assist the person supported in reaching his/her goals by being knowledgeable about the goals,
resources and supports necessary to achieve them.

10. Implement crisis prevention and intervention techniques utilizing the skills received and matching
them to the circumstances and person supported.

11. Assures documentation is completed accurately, completely and on time through role modeling
and monitoring adherence to established procedures and documentation standards.

12. Support maintenance of high-quality food, clean and comfortable shelter to promote the health
and safety of persons receiving services, protecting their personal assets and the assets of the
company.

13. Provide Person Centered Supports by facilitating what is important to the person through a
coordinated and effective and active plan of care for each person receiving services.

14. Create a positive, productive, and successful strengths-based work culture by continually
challenging and motivating employee team members to develop and improve.

15. Make great decisions and solve complex problems based upon accurate facts, by anticipating
challenges, planning for contingencies and balancing competing interests. Coach employee team
members to overcome challenges and adversities that hinder or prevent success.

16. Participate in the life of the agency by:
  • Fulfilling the mission, outcomes, policies and procedures.
  • Contributing positively to our strengths-based culture through personal learn, grow, live
    development.
Majority of work hours will be M-F, business hours. Some flexibility will required outside business hours.
Minimum Requirements: Bachelor’s degree in social work, psychology or a related field with at least two years of increasingly responsible supervisory or management experience in a health care field. Must also possess a valid driver’s license and auto insurance and have reliable transportation. Must receive favorable results from background and reference checks.
Desired: Licensure as a Registered Nursed (RN) or Licensed Practical Nurse (LPN).
Starting Salary: $46,00 - $50,000 - based on education and experience.
