Client Care:
· Provide care and services to participants according to the care plan or as approved by the LPN.
· Assists with other activities of daily living as directed by the LPN.
· Offers support and encouragement to participants and families.
· Assists with the planning, coordination, and implementation of individual and group therapeutic activities under the direction of the Life Enrichment Coordinator and/or other Day Center staff.
Reporting:
· Clearly communicates information regarding changes in participant condition to LPN.
· Demonstrates good judgment in knowing what information needs to be communicated.
· Handles communication in a timely manner.
Documentation:
· Maintains documentation of client participation in activities.
· Maintains documentation of care provided to participant.
· Records information accurately, thoroughly, and in a timely manner.
ADDITIONAL FUNCTIONS:
· Knowledgeable of the Milton Adult Day Services model, philosophy, objectives, policies and procedures.
· Arrives to work and meetings on time and ready to work.
· Respects confidentiality of clients/families and other employees.
· Attends educational meetings/conferences for professional growth as required.
· Sees change as an opportunity and maintains a level of flexibility, which allows for adaptation to new ways of performing.
· Engages participants through the use of humor, enthusiasm, and positive reinforcement.
· Performs other duties that may be assigned by LPN, Life Enrichment Coordinator, and Director.
· Requires team respect and team participation for daily demonstration of the Milton mission and model.
JOB REQUIREMENTS:
· Possesses the ability to perform the following tasks for varying periods of time: standing, walking, sitting, lifting objects up to 75 lbs., assist with 50/100 pounds, climbing stairs, stooping, kneeling, twisting, bending, reaching overhead, push or pull, simultaneous use of both hands, wrists, and fingers.
· Perform safe lifting, transferring, pushing, pull 50-75 using safe tools and techniques.
· Possess the ability to perform the following tasks for varying periods of time; speak in conversational tones utilizing English/American Language.
· Communicate with participants suffering from various disabilities (difficulty hearing/seeing, confusion, physical weaknesses, seizure disorder, Parkinson’s, etc.).
· Specific vision abilities required for this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
QUALIFICATIONS:
· Certified Nursing Assistant certification.
· Health care experience in geriatrics
