Drives referral growth and territory performance through advanced relationship management, strategic outreach, and referral source education. Serves as a trusted hospice resource to referral partners and mentors Level I representatives.
ESSENTIAL FUNCTIONS:
Strategic Relationship Management
· Develops and strengthens long-term referral partnerships with high-volume, high-influence referral sources.
· Independently manages assigned territory to meet or exceed census, admission and length-of-stay goals.
· Proactively identifies and develops new referral opportunities within hospitals, ECFs, physician practices and community agencies.
· Builds credibility with clinical and administrative referral staff, including physicians, discharge planners, case managers and social workers.
Market Growth and Hospice Education:
· Serves as a subject-matter expert on hospice eligibility, regulations and best-practice referral timing.
· Delivers advanced hospice education, including in-services, CE-supported programs and community presentations.
· Builds data strategies to communicate hospice outcomes, quality metrics and service differentiators to referral sources.
· Maintains awareness of hospice regulatory requirements, industry trends and competitor activity and shares these with referral sources regularly.
Reporting & Data Management:
· Analyzes referral and territory data to adjust outreach strategies.
· Ensures CRM data accuracy and completeness.
· Provides insight and recommendations to leadership based on territory performance.
ADDITIONAL FUNCTIONS:

· Knowledgeable in hospice philosophy and state and federal rules and regulations.
· Arrives to work and meetings on time and ready to work.
· Protects confidentiality of patients/families and other Agency employees.
· Participates educational meetings/conferences for professional growth as required.
· Sees change as an opportunity and maintains a level of flexibility which allows for adaptation to new ways of performing.
· Follows directions and policies that allow for an organizational commitment to working under time pressures without sacrificing quality.
· Performs other duties that may be assigned.
JOB REQUIREMENTS:
Possess the following abilities:
· Speak in ordinary conversational tones utilizing the English/American language
· Hear ordinary conversations
· See with the visual acuity necessary to perform the Essential Functions listed in this Job Description.
· Have simultaneous use of hands, wrists, and fingers (example: writing, typing, data entry)
· Maintains a current driver’s license.
Qualifications:
· Bachelor’s Degree preferred.
· Healthcare experience in nursing, social work, or related field desired.
· Minimum of two years sales experience, preferably in a healthcare related field.
· Requires excellent written and oral communication skills
