Job Summary:
The Supportive Apartment Counselor provides support, training, assistance and supervision to the people receiving supports in the Supportive Apartment Program.
Expectations:
Maintain positive work atmosphere by behaving and communicating in a manner that gets along with people receiving supports, collaterals, and staff.

Ensure services represent the needs and wishes of the people receiving supports and assist them to develop skills needed for independent living and inclusion in the community.
Ensure communication between all team members and supervisors regarding person’s supported.
Ensure people receiving supports are safe in their environments.
Maintain positive work atmosphere by behaving and communicating in a professional and supportive manner.
Relate the Agency’s mission statement and core values to daily responsibilities, interactions, and decision making.
Carry out assigned duties as requested by the program supervisor and or the parent/caregiver.
Responsibilities: .
- Responsible for visits to assigned apartments as per schedule.
- Provide training, counseling, assistance and guidance to people receiving supports in accordance with their goals.
- Counsel and assist people receiving supports in the areas including but not limited to: housekeeping, hygiene and grooming, clothing care, food preparation, money management, travel, shopping, use of leisure time and emergency procedures. Intervene, when necessary, on behalf of people receiving supports to resolve issues with landlords, solicitors, etc.
Monitor and inspect apartments, informing program manager of conditions related to people receiving supports’ living environments, maintenance needs, program and staffing needs, safety and security. etc. and following-up on outstanding issues.
Complete documentation including but not limited to: progress notes, billing records, and bank accounts records in accordance with agency and OPWDD standards.
Ensure people supported are independent in self-preservation skills fire safety requirements are met to include fire drills.
Provide on call services as scheduled.
Carry out any other assignments, participate in training, and attend meetings deemed necessary and appropriate to the position.
Maintain valid licenses {including driving license, as applicable], certifications or any other credentials that are required or necessary to carry out the responsibilities upon and throughout employment.
Qualifications:
A valid Driver License for a minimum of 1 year
OMH and MR/DD experience is preferred
Physical Requirements: Must be able to lift up to 20 lbs.
