We are seeking a dedicated and skilled Physical Therapist (PT) specializing in Inpatient Rehabilitation (Rehab IP) to join our multidisciplinary healthcare team. In this role, you will provide comprehensive physical therapy services to patients within a hospital inpatient setting, focusing on restoring mobility, function, and independence. Your expertise will support patient recovery through individualized care plans, effective discharge planning, and collaboration with medical teams. The ideal candidate will possess strong clinical skills, a thorough understanding of physiology and anatomy, and experience working with diverse patient populations, including geriatric, pediatric, neurological, orthopedic, and post-surgical cases.
Responsibilities
- Conduct thorough assessments of patients’ physical abilities, limitations, and rehabilitation needs upon admission and throughout treatment.
- Develop personalized care plans aligned with medical directives and patient goals, ensuring adherence to clinical documentation standards.
- Implement evidence-based therapeutic interventions such as manual therapy, trigger point therapy, gait training, and functional exercises.
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care and optimize recovery outcomes.
- Manage discharge planning processes by evaluating patient progress and recommending appropriate post-hospital services or outpatient therapies.
- Maintain accurate and timely documentation of treatment sessions in electronic health records (EHR) systems in compliance with regulatory standards.
- Educate patients and their families on injury prevention, home exercise programs, and community resources to support ongoing recovery.
- Participate in interdisciplinary team meetings to review patient progress and adjust care plans accordingly.
- Ensure compliance with hospital policies, clinical protocols, and physiotherapy best practices across inpatient rehabilitation settings.
Experience
- Proven experience practicing as a licensed Physical Therapist within inpatient hospital or rehabilitation center environments.
- Demonstrated expertise in acute care settings with familiarity in managing complex cases involving neurology, orthopedics, geriatrics, pediatrics, or home & community care.
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques relevant to inpatient rehabilitation.
- Prior experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation management.
- Familiarity with outpatient clinic or outpatient rehabilitation center experience is advantageous.
- Knowledge of discharge planning procedures and collaboration with outpatient clinics or home health services.
- Ability to work effectively with diverse patient populations across various age groups while maintaining high standards of patient care.
Required Education: Bachelor's in Physical Therapy.
Required Certifications & Licensure: IL PT License, BLS,AHA.
