The Position
As a Nurse Practitioner, you will serve as the dedicated clinical lead for a single Skilled Nursing Facility (SNF), leveraging AI-powered insights to proactively manage patient care and improve clinical outcomes. This full-time, field-based role is based onsite at a dedicated facility in Clarksville, Goodlettsville, or Gallatin, Tennessee. You will be responsible for conducting comprehensive patient assessments, managing plans of care, responding to AI-generated clinical alerts, and collaborating closely with facility staff to reduce preventable hospitalizations and improve patient outcomes.
Primary responsibilities include:
- AI-driven patient prioritization: Review predictive alerts and patient risk data daily to identify high priority patients and coordinate interventions with nursing staff.
- Conduct patient assessments: Perform comprehensive E&M visits, evaluate new admissions, assess
- changes in condition, reconcile medications, and initiate or adjust treatment plans.
- Manage clinical programs: Support cardiopulmonary monitoring, high-risk patient management, post hospitalization follow-up, and AI-triggered rapid response initiatives.
- Leverage AI-powered insights: Utilize predictive trend data to guide clinical decision-making and support proactive patient interventions.
- Collaborate with facility teams: Partner with Directors of Nursing, nursing staff, and interdisciplinary
- teams to coordinate patient care and ensure timely interventions.
Qualifications and Candidate Background
- Certified and licensed Nurse Practitioner in the state of Tennessee, with ANCC (or equivalent)
- certification in Adult, Family, Geriatric, or Acute care.
- 2 years of experience caring for complex adult populations in Skilled Nursing Facilities (SNF), Long-Term Care (LTC), or post-acute care settings.
- Ability to practice independently, including initiating and adjusting plans of care, placing orders, and managing patient treatment plans.
- Proficient with healthcare technology, including EMRs, Google Suite, Slack, and AI tools.
