We are seeking a compassionate and skilled RN Case Manager to provide exceptional home health care throughout the Ypsilanti and greater Washtenaw service area, including:

• Ypsilanti
• Ann Arbor
• Saline
• Belleville
• Canton
• Superior Township
• Pittsfield Township
• Van Buren Township
• Milan
• Dexter (as needed)

This role is ideal for an RN who thrives on connection, clinical independence, and helping patients improve their health and quality of life at home.

In This Role, You Will

• Conduct comprehensive nursing assessments
• Develop individualized plans of care
• Deliver skilled nursing interventions in the home
• Educate patients and families on condition management and medications
• Coordinate with physicians, therapists, and interdisciplinary team members
• Complete accurate and timely documentation in WellSky / Kinnser
• Utilize Home Health Notify for real-time communication and updates
• Ensure compliance with state, federal, CHAP, and Joint Commission standard

You’re a Great Fit If You

• Graduated from an accredited school of nursing
• Hold an active Michigan RN license
• Have at least 2 years of nursing experience
• Home health experience preferred
• Are compassionate, resourceful, and organized
• Have excellent clinical judgment and communication skills
• Have reliable transportation, valid auto insurance, and a driver’s license
