We’re looking for an RN Case Manager who thrives on connection, critical thinking, and helping patients live their best possible lives to provide care across the greater Flint area (Lapeer/Davison/Fenton/Swartz Creek, etc.)

In this role, you will:

• Conduct thorough nursing assessments and create individualized care plans.
• Deliver skilled nursing interventions in the home.
• Teach patients and families how to manage health conditions and medications.
• Coordinate with physicians, therapists, and other team members to ensure seamless care.
• Keep documentation accurate, timely, and compliant with state, federal, CHAP, and Joint Commission regulations.

You’re a great fit if you:

• Graduated from an accredited school of professional nursing.
• Hold a current Michigan RN license.
• Have at least two years of nursing experience (home health preferred).
• Are compassionate, resourceful, and an excellent communicator.
• Have a valid driver’s license, insurance, and reliable transportation.
