We are seeking a compassionate and skilled RN Case Manager to provide exceptional home health care throughout West Oakland County, including:

In This Role, You Will
- Conduct comprehensive nursing assessments
- Develop individualized plans of care
- Deliver skilled nursing interventions in the home
- Educate patients and families on disease management and medications
- Coordinate with physicians, therapists, aides, and interdisciplinary team members
- Complete accurate and timely documentation in WellSky / Kinnser
- Utilize Home Health Notify for real-time team communication
- Ensure compliance with state, federal, CHAP, and Joint Commission standards

You’re a Great Fit If You
- Graduated from an accredited school of nursing
- Hold an active Michigan RN license
- Have at least 2 years of nursing experience
- Home health experience preferred
- Are compassionate, resourceful, and highly organized
- Have strong clinical judgment and communication skills
- Have reliable transportation, auto insurance, and a valid driver’s license
