Job Title – Registered Nurse (RN) – Case Manager
Unit – Long Term Care
Duration of Assignment – 13 Weeks
Location – /Hermiston, Oregon
Shift – Day Shift (8 Hours, 5 Days/Week)
Guarantee Hours – 40 Hours per Week
Salary Range – Up to $114/Hour
Job Duties:
• Coordinate and manage patient care plans across the continuum of care.
• Assess patient needs and develop discharge and care transition plans.
• Collaborate with physicians, nurses, and interdisciplinary teams.
• Monitor patient progress and ensure appropriate resource utilization.
• Facilitate referrals and coordinate services with external providers.
• Maintain accurate documentation and ensure compliance with regulations.
Requirements:
• Active RN license required.
• Case Management experience preferred.
• Strong knowledge of care coordination and discharge planning.
• Excellent communication and organizational skills.
• Ability to work independently and manage multiple cases.
Salary : $100 - $114
