Job Title: Nurse - LPN (Evening Shift)
Location: Elmore Assisted Living
Schedule: Part-Time or Full Time Evenings and Weekends
Pay: $30-$40 per hour
Weekend Shift Differential: $1.00 per hour additional on the weekends.

Nurse - RN / LPN Overview:
- Oversee the coordination and plan of residents' care, ensuring care practices have each resident's best interests in mind
- Complete clinical assessments of residents' health status and document appropriately
- Monitor the status of chronic illnesses and manage the administration of medications
- Coordinate with external health care providers and internal team members
- Interact with residents' families and loved ones, reporting changes in their health or care plan
Nurse – RN / LPN Qualifications:
- Licensed as a Registered Nurse / RN or Licensed Practical Nurse / LPN in Minnesota
- Experience with Electronic Medical Records (EMR) software
- Prior nursing experience in a SNF, AL, TCU, or Home Health setting is ideal
- Demonstrated ability to work autonomously
- Excellent at planning, organizing, and communicating
