What You’ll Do
• Assist with daily operations of the Psychosocial Rehabilitation (PSR) program
• Conduct registrations, assessments, and documentation in the electronic health record (EHR)
• Develop and implement individualized treatment plans and monitor progress
• Facilitate group sessions focused on social, educational, and life skills development
• Maintain accurate progress notes and attendance records
• Collaborate with psychiatrists, nurse practitioners, case managers, and care teams
• Provide education to clients and families regarding health and medications
• Ensure client safety, confidentiality, and compliance with all standards
• Support program quality and participate in team meetings and initiatives
What You Bring
• Bachelor’s degree in Counseling, Social Work, Psychology, or related human services field
Preferred
• Experience in behavioral health, PSR, or day treatment settings
• Strong group facilitation and engagement skills
• Knowledge of treatment planning and documentation standards
• Effective communication and teamwork abilities

Additional Requirements
