We are seeking a dedicated and patient-focused Pharmacist to join our on-site pharmacy team within a Federally Qualified Health Center (FQHC). This role is ideal for a pharmacist who is passionate about improving access to care, supporting underserved populations, and working collaboratively in an integrated healthcare setting.
Key Responsibilities:

Dispense prescription medications accurately and in compliance with federal, state, and FQHC regulations

Provide patient-centered medication counseling, education, and adherence support

Collaborate with providers, nurses, and care teams to optimize medication therapy and outcomes

Participate in chronic disease management and population health initiatives

Maintain accurate pharmacy records and documentation

Supervise pharmacy technicians and support staff as needed

Pharm D from an accredited pharmacy school of pharmacy

Active and unrestricted pharmacist license in Ohio

Previous experience in ambulatory care, community pharmacy, or FQHC setting preferred

Previous experience in behavioral healthcare strongly preferred

Knowledge of 340B pharmacy program strongly preferred
