Homeless Outreach Engagement Specialist

Southeast Healthcare has been awarded a new grant in collaboration with The City of Columbus to strengthen outreach to unsheltered residents with the highest needs, with repeated interactions with first responders who often experience mental health and/or substance use challenges. This initiative supports a broader strategy to stabilize housing for all residents and reduce strain on emergency and outreach systems.

Responsibilities:

• Conduct outreach and engagement with referred unsheltered residents.
• Work in coordination with city staff and first responders to reach city residents.
• Identify goals and assist with barrier elimination to stable living.
• Provide services and/or referrals to temporary housing/lodging programs.
• Maintain accurate and complete records.

Shifts available: 

       Full-time, evening shift. 

       Part-time, weekends.

Requirements:

• CDCA required. 
• Associate or bachelor's degree strongly preferred.
• Strong understanding of homelessness required.
• Experience with SPMI and/or SUD population preferred.
• Data collection/outcome achievement experience preferred. 
• Strong interpersonal and communication skills.
• Ability to work effectively as part of an interdisciplinary team.
• Crisis intervention and de-escalation skills.
• Knowledge of community resources and healthcare services.
• Valid driver's license and insurance.
