About the Role
We are seeking a Referral Navigator to support patient access to specialist care by managing, processing, and coordinating medical referrals within a healthcare network.
This role ensures patients are efficiently connected to the right specialists by validating referral documentation, managing workflows, and coordinating across primary care providers, specialists, and internal care teams.
You will play a key role in improving care continuity, referral accuracy, and patient access to timely treatment.
Key Responsibilities
Referral Processing & Coordination
- Build and process new referrals from primary care providers
- Review referral documentation for completeness and accuracy
- Return incomplete referrals for additional information
- Cancel or escalate referrals that do not meet criteria
- Forward complex or unclear cases for medical review
- Determine appropriate routing (internal, external, overflow, or outsource providers)
Administrative & Documentation Management
- Handle high volumes of paperwork, faxing, scanning, and electronic documentation
- Maintain accurate records within electronic medical systems
- Ensure referral workflows meet compliance and timeliness standards
- Support scheduling and coordination of specialist appointments
Patient & Provider Communication
- Communicate with providers, clinics, and internal teams via phone and electronic systems
- Provide updates and clarification regarding referral status
- Engage in direct patient interaction when required
- Maintain professional, clear communication across all touchpoints
Work Environment
- Primarily desk-based, computer-heavy role (>90% system work)
- High volume of phone and documentation handling
- Direct interaction with patients and healthcare providers
- Structured weekday schedule (no weekends)
Requirements
Essential
- High school diploma or GED
- 2 years experience in a healthcare-related environment
- Experience using Electronic Medical Records (EMR) or practice management systems
- Ability to obtain CPR certification within 30 days of hire
- Strong communication and telephone etiquette skills
Preferred
- Experience as a Medical Assistant or in clinical support roles
- Familiarity with referral management or care coordination workflows
- Bilingual (English/Spanish) is an advantage
