Position Summary
The Rapid Response Case Manager is responsible for all case management aspects of the Rapid Response Homelessness Prevention program. This includes assessing client needs, developing individualized service plans, providing ongoing support and advocacy, and ensuring clients have access to necessary resources and services. Additionally, the position will provide support in administering Rapid Response funding as needed, which involves assisting with budgeting, tracking expenditures, and ensuring compliance with funding guidelines to effectively support clients in maintaining stable housing.
Essential Functions, Duties and Responsibilities
- Conduct comprehensive applicant interview and assessment process
- Meet with individuals and families at the office on a regular basis- home visits may be required on an as needed basis
- Provide crisis intervention and support to clients
- Develop, monitor and implement individual service plans (ISP) for clients throughout course of working together
- Complete all funding source paperwork and maintain accurate and updated client files (digital and physical files)
- Refer and assist with access to mainstream resources and benefits to address unmet needs (including public benefits, shelter, healthcare, health insurance, recovery programs, mental health supports, legal services, employment/education services, childcare, budgeting resources, parenting classes, credit counseling, etc.)
- Work with clients throughout the case management process by providing information, support, and advocacy while tracking their progress toward their goals
- Assist with budgeting and credit repair needs; benefit attainment; access to healthcare; addressing court related issues; and medication monitoring; parenting education; connections to education, employment, and volunteer opportunities; referrals to mainstream services; and activities related to family, social and community reintegration
- Ensure housing lease compliance and liaise with landlords as necessary to maintain positive relationships
- Enter comprehensive individual meeting notes, assessments and collateral contacts into online database system (HMIS)
- Assist with client housing search and relocation
- Attend individual supervision, staff meetings, and community-based meetings
- Perform other duties to ensure overall smooth operation of all agency programs
- Obtain all required verifications and other related documentation to complete application process
- Screen applications to determine preliminary eligibility for individual funding sources
- Meet in person with applicants to collect required information to determine eligibility
- Prepare application files in compliance with funding requirements for supervisor review
- Send out letters informing applicants of their application status
Education, Abilities, Competencies and Experience Required
- Fluent in Spanish or Portuguese required
- Ability to communicate effectively—orally and in writing
- Demonstrated ability to cultivate and maintain positive working relationships
- Working knowledge of resources and services available to the homeless
- Excellent organizational skills and detail oriented
- Ability to manage multiple tasks and adhere to strict deadlines
- Excellent time management skills
- Skilled in Word, Excel and other web-based applications (Google Applications)
- Ability to take initiative, follow-through consistently and work independently
- Proven ability to function as a strong advocate for clients
- Proficient computer skills with email, data entry, word processing and spreadsheets
- Highly organized
Supervisory Responsibility
This position does not have supervisory responsibilities.
Work Environment
This job operates in an office and professional environment. This role routinely uses standard office equipment such as computers, phones, photocopiers. On occasion the job will require going out in events or organizations, based on the needs of the business.
Physical Demands
This is mostly sedentary outside of travel in an office space. This would require the ability to sit at a desk, frequent walking, bend or stand as necessary.
Position Type and Expected Hours of Work
Designated days and schedule will be primarily Monday-Friday though will also be based on the needs of the operation and business.
Travel
Position requires limited travel to VSPs in the CoC; based on the needs of CoC operations and SHC business.
