Responsibilities:
- Provide direct patient care within the SNF environment following care plans and facility protocols.
- Monitor and record patient conditions, assessments, and vital signs.
- Administer medications and treatments according to physician orders.
- Assist residents with ADLs to support high-quality long-term care.
- Maintain accurate documentation in the EMR system.
- Communicate effectively with the care team to support strong clinical outcomes.
What You’ll Need to Succeed:
- Active GA LPN license.
- Current BLS certification.
- Minimum 1 year of LPN experience (SNF or long-term care experience preferred).
