Case Worker - OAC
Status: Full-Time
Compensation: $24.73 per hour
Location: Forest Hills, Jamaica, Kew Gardens, and Pomonok, Queens, NY
Schedule: Monday through Friday (varies)
The Position
- Interview clients to assess challenges related to personal and family adjustment, finances, housing, and physical or mental health concerns.
- Provide accurate and appropriate information regarding available services, benefits, and entitlements.
- Conduct on-site visits with new program participants to support engagement and integration into center activities.
- Assist clients with benefit applications and recertification processes.
- Develop case notes and action plans that reflect participant assessments and goals; follow up on outcomes and next steps.
- Complete new client registrations and maintain accurate participant records.
- Make telephone reassurance and wellness calls to participants.
- Collaborate with program directors and staff to ensure participant needs are addressed through culturally responsive and effective service delivery.
- Build and maintain partnerships with community organizations and service providers to expand available resources.
- Advocate and intervene on behalf of participants with external agencies when appropriate.
- Participate in staff meetings, trainings, conferences, and professional development activities.
- Perform other duties as assigned.
- Associate's degree in Human Services, Social Work, Gerontology, Psychology, or a related field preferred; equivalent experience considered.
- Minimum of one year of experience providing case management, social services, or direct support to older adults or vulnerable populations.
- Bilingual proficiency required.
- Knowledge of public benefits, entitlement programs, and community resources.
- Strong interpersonal, organizational, and documentation skills.
- Ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds.
- Proficiency with Microsoft Office and electronic record-keeping systems.
- Ability to travel between multiple center locations throughout Queens.
