Job Title: Case Manager

Status: Full-Time, Non-Exempt

Compensation: $65,200 annually

Location: Austin Street Site, Queens, NY

Schedule: Monday - Friday; 35 hours per week

Position Summary

Queens Community House is seeking a compassionate and dedicated Case Manager to join our Case Management Program serving older adults. This position plays a critical role in helping older adults with functional impairments remain safely in their homes by connecting them to essential services, benefits, and community resources.

The Case Manager will maintain an average caseload of approximately 70 older adults residing in Community Districts 4 and 6, including Elmhurst, Corona, Forest Hills, and Rego Park. Through comprehensive assessments, care planning, advocacy, and ongoing support, the Case Manager will empower clients to maintain their independence and quality of life.

 

Primary Responsibilities

  • Conduct intake screenings and assess prospective clients' eligibility for case management services.
  • Perform in-home assessments and reassessments to determine service needs and continued eligibility.
  • Develop individualized, culturally responsive care plans based on client needs and available community resources.
  • Assist clients in accessing benefits, entitlements, and supportive services.
  • Build collaborative relationships with clients, caregivers, and informal support networks.
  • Coordinate services with external providers and advocate on behalf of clients.
  • Maintain accurate, complete, and timely case documentation.
  • Collaborate with QCH colleagues and make internal referrals as appropriate.
  • Coordinate Supplemental Services Fund requests, including identifying client needs and assisting with the purchasing and delivery of approved items.
  • Participate in community outreach efforts as needed.
  • Complete all required NYC Aging and agency training requirements.
  • Attend staff meetings, supervision sessions, annual evaluations, and staff development activities.
  • Work collaboratively as a member of the Case Management team.
  • Perform other duties as assigned.

 

Qualifications

  • Bachelor's degree required.
  • Minimum of two (2) years of experience working with older adults.
  • MSW or related Master's degree (Social Work, Public Administration, Nursing, Public Health, or related field) preferred.
  • Bilingual Korean required.
  • Russian language proficiency preferred (Korean and Russian language skills are highly desirable).
  • Demonstrated cultural competency and sensitivity to the diverse needs of older adults.
  • Knowledge of communication strategies for individuals with visual and hearing impairments.
  • Strong organizational and multitasking skills.
  • Proficiency with Microsoft Office and other computer applications; experience with the Senior Tracking, Analysis, and Reporting System (STARS) database is a plus.
  • Ability to maintain confidentiality and exercise sound judgment.
  • Strong analytical and problem-solving skills.
  • Excellent interpersonal, communication, and customer service skills.
  • Proven ability to manage multiple priorities and follow through on assignments.
