Community Services Specialist in Androscoggin County. This is a

full-time, hybrid position (40 hours/week) where youll make a real

impact supporting older adults, adults with disabilities, and their

families.**

As a team member of the Community Services Department,

• responsibilities include**:
• Providing information, advocacy, counseling, and assistance to older
  
  

adults, adults with disabilities, and their families.

• Supporting clients in identifying and accessing community-based
  
  

services and in planning for long-term support needs.

• Performing comprehensive client assistance services, including
  
  

information sharing, needs assessment, education on long-term care

options, and future planning.

• Providing counseling and referrals, ensuring clients and families
  
  

are linked to needed services.

• Advocating on behalf of clients and promoting community awareness of
  
  

long-term care planning.

• Serving as a collaborative liaison with community health and human
  
  

service organizations.

Minimum Qualifications

• Bachelors degree in social services, human services, social work, or
  
  

a related field required.

• Equivalent combination of education and relevant experience may be
  
  

considered.

• Licensed Social Worker (LSW) or eligibility for licensure preferred.
• 13 years of professional experience in case management, social work,
  
  

nursing, gerontology, health services, or related human services

field preferred.

• Valid Maine State drivers license.
• Must receive favorable results from driver\'s license, reference,
  
  

and criminal conviction background checks for any offer of

employment.

\$23.45 - \$28.14/per hour (based on qualifications and experience)
