Duties
- Conduct comprehensive patient assessments to develop individualized care plans
- Collaborate with interdisciplinary teams to coordinate patient care and facilitate effective communication among healthcare providers
- Manage discharge planning processes, ensuring patients receive appropriate follow-up care and resources post-discharge
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs
- Advocate for patients by providing education on their conditions, treatment options, and available resources
- Maintain accurate documentation of patient interactions, care plans, and outcomes in compliance with healthcare regulations
- Stay informed about best practices in managed care and apply them to improve patient outcomes
- Current Registered Nurse (RN) license in the State of Missouri
- Demonstrated ability in patient assessment and case management practices
- Knowledge of discharge planning processes and managed care principles is highly desirable
- Strong interpersonal skills with the ability to work collaboratively within a team environment
- Excellent organizational skills and attention to detail in managing multiple cases simultaneously
