Schedule: This position is part of the Nursing Management Team and will regularly work Monday through Friday with participation in the on-call rotation. This position must be willing to work evenings and weekends as needed.

Role:
We are seeking a dedicated Care Coordinator to join our team! The Care Coordinator assists the Director of Nursing in the supervision and administration of the quality of nursing care for Scenic Manor. The Care Coordinator is responsible for completion of the MDS in support of the MDS Coordinator. The Care Coordinator will be responsible for preadmission reviews, admissions and readmissions. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.
Responsibilities:
- Assist with completion of MDS 3.0 and care plans.
- Work closely with the Marketing Director to review admission inquiries and perform preadmission assessments to ascertain care needs and prepare for move ins.
- Complete process for preadmission and admission of residents including PASRR, orders, assessments and other required documentation.
- Assists in all phases of the delivery of nursing care for residents in the nursing facility.
- Manage readmission orders.
- Understands, practices and communicates resident directed/centered care on a daily basis as it relates to the nursing department.
- Review provider orders for accuracy completion.
- Review medical records for items that need follow up and address any discrepancies.
- Participates in on-call rotation with other nurse leadership team members.
Qualifications:
- The CareCoordinator must be a licensed nurse from an accredited college, be currently licensed in state of which applying without encumbrance, be responsible for maintaining professional licensure and CEU requirements.
- Current CPR certification required.
- A registered nurse is preferred with 1- 2 years supervisory experience in a geriatric setting.
