Range $2600 Weekly Gross pay (Non-taxable
travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage
Reimbursement: $0.50/mile for distance
covered between patient home appointments
Unit: Home Health
Shift: 8 Hours; Days, 8a – 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri.
Position Requirements:
- Registered
Nurse: 2 years - Required
- Recent/Current
Home Health Nursing experience: 1 year within the last 3 years - Required
- Home Care Home
Base (HCHB) charting experience - Required
- Must have valid
Driver’s License and Personal Auto Insurance
- Must be Covid
Vaccinated or provide medical or religious exemption
Education, License
and Certification Requirements:
- ASN or an
equivalent – Required
- BSN or an
equivalent – Highly preferred
- RN License for
State of Oregon - Required
- BLS
Certification from AHA or American Red Cross - Required
Job Description:
- Provide complete and comprehensive nursing care for home health patients at their home.
- Completes an initial, comprehensive and ongoing comprehensive
assessment of patient and family/caregiver living situation to determine care
- Provides a complete physical assessment and history of current and previous
illness(es).
- Provides professional nursing care by utilizing all elements of nursing
process and as defined in the state Nurse Practice Act.
- Assesses and evaluates patient’s status by establishing the initial individualized plan of care in collaboration
with the certifying medical provider based on comprehensive assessment and
patient goals.
- Regularly re-evaluating patient and family/caregiver needs.
- Participating in revising the plan of care as necessary in
collaboration with the certifying medical provider as patient status and needs
change.
- Administers medications and treatments as
prescribed by the medical provider in the plan of care.
- Involves the patient and family/caregiver to address needs and meet
related goals.
- Provides health care instructions to the patient as appropriate per
assessment and plan.
- Facilitates the patient’s efforts toward self-sufficiency or obtaining
appropriate care giving.
- Acts as Case Manager when assigned by Clinical Manager and assumes
responsibility to coordinate patient care for assigned caseload.
