Position Requirements:
- LPN experience: 2
years - Required
- Recent/Current
Home Health LPN Experience: 1 year within the past 3 years – Required
- Home Care Home Base
(HCHB) charting experience: Recent/current - Required
- Must be Covid
Vaccination with booster - No exemptions, booster can be obtained upon offer
Education, License
and Certification Requirements:
- LPN License for State
of VA - Required
- BLS Certification
from AHA or American Red Cross - Required
Job Description:
- Under general supervision, provide and
coordinate care for assigned Home Health patients
- Empower patients and
families to reach healthcare goals by educating them on disease management.
- Under general supervision, provide Home Health nursing
care and services within the standards and scope of nursing practice as
defined within the state Nurse Practice Act regulations, policies, procedures,
and protocols, as well as facility and corporate policies and procedures.
- Under general supervision, ensure physicians’
orders are legible if taken verbally and transcribed appropriately per
policy and procedure.
- Under general supervision, ensure patients’
status is assessed on an ongoing basis and pertinent information is
gathered and documented.
- Notify physicians and/or supervising clinician of
significant changes in status.
- Document and complete
charting
- Territories: Fairfax & Prince William County area and 50 miles radius
Salary : $1,680 - $1,760
