Pay Rate:
Range $2520 to $2600 Weekly Gross pay based on a 40-hour work week (Includes non-taxable
travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage
Reimbursement: $0.49/mile for distance
covered between patient home appointments
Unit: Home Health Hospice
Shifts available: Days: 8 Hours;
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri
Position Requirements:
- Registered Nurse: 1 year - Required
- Recent/Current Hospice experience in a Home Health setting: 1 year within the last 3 years – Required
- Case Management and admissions experience - Required
- Home Care Home Base
(HCHB) charting - Required
- Must have valid
Driver’s License and Personal Auto Insurance
Education, License
and Certification Requirements:
- ASN or an equivalent
– Required
- BSN or an equivalent
– Highly preferred
- RN License for State
of the state applying or Compact if acceptable - Required
- BLS Certification
from AHA or American Red Cross - Required
Job Description:
- Provide and
coordinate care and case management for assigned hospice patients in comfort of their homes.
- Develop and implement
nursing care plans including SOC, and all types of visits as assigned.
- Performs Case Management, Admissions and all types of visits for the assigned patient case load
- Support patient comfort, pain management and ability to stay at home - enabling them to spend quality time
with their loved ones.
- Empower patients and
families by educating them on disease management and pain management.
- Provide bereavement support.
- Provide Hospice nursing
care and services within the standards and scope of nursing practice as
defined within the state Nurse Practice Act regulations, policies, procedures,
and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’
orders are legible if taken verbally and transcribed appropriately per
policy and procedure.
- Ensure patients’
status is assessed on an ongoing basis and pertinent information is
gathered and documented.
- Notify physicians of
significant changes in status.
- Document and complete
charting for all patient visits within 24 hours.
- Provide bereavement
support to patient’s families as needed.
