Role Overview: Provides comprehensive nursing care in a long-term care setting by developing and implementing individualized care plans, assessing residents' physical, emotional, and psychological needs, and coordinating with healthcare teams.
Key Responsibilities: Review physician prescriptions, communicate care plans, and support residents and families. Develop care plans based on medical histories and assessments, educate residents and families on health management, and evaluate care outcomes. Ensure a safe environment by adhering to infection control, medication, and safety protocols. Maintain and troubleshoot care equipment and supplies, coordinate admissions, discharges, and transfers, and supervise nursing staff workflow.
Qualifications & Skills: Active RN license in the practice state, with preferred 1-3 years of clinical or long-term care experience. Must have effective communication skills, visual acuity, and the ability to work as part of a team.
