Position Summary
The Swing Bed Assistant / Activities Coordinator is a dual-role position responsible for providing direct patient care as a Certified Nursing Assistant (CNA) while coordinating and leading therapeutic, recreational, and social activities for patients participating in the Swing Bed Program. This role supports patient recovery, promotes independence, and enhances quality of life through compassionate care and meaningful engagement opportunities. The position works closely with patients, families, providers, and interdisciplinary team members to create a positive and supportive healing environment.

Qualifications

High school diploma or equivalent required.

Current Louisiana Certified Nursing Assistant (CNA) certification required.

Current Basic Life Support (BLS/CPR) certification required.

Experience providing direct patient care required.

Strong communication, interpersonal, and customer service skills.

Ability to work independently and collaboratively as part of a healthcare team.

Strong organizational and time management skills.

Ability to maintain patient confidentiality and professionalism at all times.

Preferred Qualifications

Experience in long-term care, swing bed, rehabilitation, or skilled nursing settings.

Experience coordinating activities, recreation programs, or patient engagement initiatives.

Knowledge of geriatric care and rehabilitative care principles.

Key Responsibilities

Provide direct patient care under the direction of licensed nursing staff.

Assist patients with activities of daily living, including bathing, dressing, grooming, feeding, toileting, and mobility.

Monitor patient comfort, safety, and well-being and report changes in condition to nursing staff.

Plan, coordinate, and facilitate therapeutic, recreational, educational, and social activities for Swing Bed patients.

Encourage patient participation in activities that support recovery, independence, and overall wellness.

Adapt activities to meet the physical, emotional, cognitive, and social needs of individual patients.

Maintain activity schedules, participation records, and required documentation.

Collaborate with nursing staff, rehabilitation services, providers, and families to support individualized patient goals.

Assist with patient transportation and preparation for activities as needed.

Observe professional ethics and maintain strict confidentiality of all patient, family, medical, financial, and employment information in accordance with HIPAA regulations and Riverside Medical Center policies.

Follow accepted patient safety practices and organizational policies.

Participate in required safety training and educational programs.

Perform additional duties as assigned.
