River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. River Edge is actively seeking a full-time Case Manager to join our Core Services Department. The ideal candidate will play a crucial role in providing a broad range of paraprofessional social services to participants and their families, ensuring alignment with treatment, service, and aftercare plans. Key responsibilities include crisis interventions, maintaining current medical records, and working collaboratively with patients, families, physicians, and nurses to ensure high-quality care.

Duties and Responsibilities
- Provide a broad range of paraprofessional social services to participants and their families, including direct services and resource connection that align with treatment/service/aftercare plans.
- Performs crisis interventions as needed/required.
- Maintains/keeps current required medical records.
- Work collaboratively with patients, families, physicians, and nurses to ensure high quality care.
- Refers clients to needed services and resources.
- Works as a liaison with client and resources.
- Support a maximum of 25 participants.
- Communicate client needs to a variety of care team members and follow up accordingly.
- Prepare and submit referrals for clients for varying services and resources such as housing, treatment facilities, obtaining identification, etc.
- Coordinate and attend client appointments, assist with transportation needs, conduct scheduled home visits, face to face and case management contact calls.
- Attend meetings and training courses as scheduled.
Qualifications
- Bachelor's degree from an accredited college or university with a major in social services related disciplines (social work, sociology, psychology).
- Previous experience working in a behavioral health care setting is preferred.
- Must have a valid Georgia Driver's License and a good driving record.
- Provide 2 Professional references (Former employers only, no family members),
