What are the responsibilities and job description for the Service Planner position at Rehabilitation Support Services, Inc.?
Our Certified Apartment Program has 2 openings for a Service Planner. This is a full time position and the hours are Monday – Friday 8AM-4:30PM (hours are somewhat flexible).
Minimum qualifications for Service Planner:
- High School diploma or equivalent
- 1 year experience in the Human Services field
- Valid NYS driver's license required
- Ability to type, and to write clearly and objectively
Service Planner responsibilities include:
- Collaboration with staff and clients on developing service plans according to OMH and Medicaid regulations
- Review of weekly contact notes
- Perform quarterly chart audits
- Completion of documentation according to agency standards
- When down staffed, coverage of clients is required.
