Rehabilitation Support Services, Inc. (RSS) is seeking a compassionate and motivated Housing Specialist to join our new ESSHI (Empire State Supportive Housing Initiative) program in Albany, NY.
This newly developed, 60-unit apartment building includes 8 supportive housing units for individuals experiencing homelessness who have behavioral health diagnoses and 3 supportive housing units for individuals experiencing homelessness who have substance use diagnoses.
As a Housing Specialist, you will provide individualized case management and supportive services to a caseload of 11 tenants, helping them achieve housing stability, increase independence, and successfully integrate into the community.
Responsibilities
The Housing Specialist is responsible for providing person-centered support, including but not limited to:
- Assisting individuals in securing benefits and entitlements.
- Connecting tenants with medical, behavioral health, and community resources.
- Supporting tenants in establishing and maintaining a safe, clean, and comfortable living environment.
- Assisting with budgeting and money management skills.
- Providing medication supervision and support as appropriate.
- Monitoring health concerns and coordinating with treatment providers.
- Teaching and strengthening daily living and independent community living skills.
- Developing and monitoring individualized service plans.
- Maintaining accurate documentation and case records.
- Participating in the agency's supportive housing on-call rotation.
Schedule
- Full-time (40 hours per week)
- Tuesday–Saturday, 8:00 a.m.–4:30 p.m.
- On-call responsibilities for one week every 8–12 weeks as part of the Supportive Housing on-call system.
Qualifications
- High school diploma or equivalent required.
- Valid New York State driver's license required.
- CASAC certification and/or experience working with individuals with behavioral health or substance use disorders preferred.
- Strong communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
