We are seeking a dedicated and experienced Psychiatric Practitioner (Psychiatric Nurse Practitioner, Psychiatrist, or other qualified psychiatric prescriber) to serve as an integral member of our multidisciplinary community-based behavioral health teams, including Assertive Community Treatment (ACT) and Safe Options Support (SOS).

This position provides comprehensive psychiatric care to individuals with severe and persistent mental illness, co-occurring disorders, and complex behavioral health needs. The Psychiatric Practitioner delivers person-centered, recovery-oriented services in community settings, including client homes, shelters, clinics, and other natural environments. The role combines direct psychiatric treatment, medication management, clinical leadership, care coordination, crisis intervention, and team consultation to support individuals in achieving stability, recovery, and community integration.

Essential Duties and Responsibilities

Direct Clinical Care

  • Conduct comprehensive psychiatric evaluations, diagnostic assessments, and ongoing psychiatric treatment.
  • Develop, implement, and revise individualized treatment plans in collaboration with clients and multidisciplinary team members.
  • Provide medication management, including prescribing, monitoring effectiveness, managing side effects, and adjusting treatment as clinically indicated.
  • Deliver psychotherapy and other evidence-based psychiatric interventions as appropriate.
  • Provide education to individuals and families regarding diagnoses, treatment options, medications, symptom management, and recovery strategies.
  • Utilize flexible treatment approaches, including community-based visits, engagement encounters, telepsychiatry, and outreach services to improve access to care.

Specialized Psychiatric Services

  • Administer and monitor long-acting injectable medications.
  • Provide Medication Assisted Treatment (MAT) services when clinically appropriate.
  • Implement and monitor clozapine therapy.
  • Support medication delivery and adherence strategies for individuals with significant treatment barriers.
  • Coordinate psychiatric care during transitions between hospitals, crisis services, residential programs, and community settings.

Team Collaboration and Care Coordination

  • Participate in daily multidisciplinary team meetings to review client status, coordinate interventions, and ensure continuity of care.
  • Collaborate closely with nurses, clinicians, case managers, peer specialists, and community providers to deliver integrated care.
  • Consult with outside psychiatric providers, primary care practitioners, hospitals, crisis services, and specialty providers to coordinate treatment and clarify clinical roles.
  • Promote integration of physical and behavioral health services through ongoing communication with healthcare partners.
  • Assist in crisis assessment, intervention, and stabilization efforts as part of the team's comprehensive service model.

Clinical Leadership and Education

  • Provide psychiatric expertise and clinical leadership to the ACT and SOS teams.
  • Collaborate with team leadership to develop and implement evidence-based treatment practices.
  • Support staff development through consultation, mentoring, and clinical training.
  • Identify clinical education needs and provide ongoing instruction related to psychiatric care, medications, diagnoses, and treatment approaches.
  • Review quality of care outcomes and participate in continuous quality improvement initiatives.
  • Provide recommendations regarding case management and service delivery based on participants' psychiatric and clinical needs.

Documentation and Compliance

  • Complete timely and accurate clinical documentation in accordance with agency policies, regulatory standards, and billing requirements.
  • Maintain compliance with all state, federal, and professional practice standards.
  • Monitor client progress, identify risks, and adjust treatment interventions accordingly.

Qualifications Required

  • Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP), Psychiatrist (MD/DO), or other qualified psychiatric prescriber eligible to practice in New York State.
  • Strong clinical assessment, diagnostic, medication management, and crisis intervention skills.
  • Ability to work independently and collaboratively within multidisciplinary teams.
  • Excellent communication, documentation, and organizational skills.
  • Valid driver's license and ability to travel throughout the community.

Preferred

  • Experience working with individuals experiencing severe and persistent mental illness, co-occurring disorders, homelessness, and high-service needs.
  • Familiarity with Assertive Community Treatment (ACT), intensive community-based treatment, or mobile behavioral health models.
  • Experience with long-acting injectable medications, Medication Assisted Treatment (MAT), and clozapine management.
  • Commitment to recovery-oriented, person-centered, and trauma-informed care practices.

Work Environment

This position is stationed out of Saratoga, with clients also in Albany and Schenectady counties. Services are delivered in client homes, shelters, community locations, healthcare facilities, and other natural environments. The role requires flexibility, collaboration, and comfort working with diverse populations experiencing complex clinical and social challenges.
