Location: Northern Dodge County
Position Type: Full Time Salaried
Schedule Requirements: Primarily day shift, but also includes a required on-call, weekend, and holiday rotation
Sign-On Bonus: $6,000 - $10,000 based on hospice experience

Purpose of the Position
Due to exponential growth and increased demand in Northern Dodge County, Rainbow Community Care is seeking a dedicated Registered Nurse Case Manager to help build this additional service area. The ideal candidate for this position will reside in Dodge County.
Key Responsibilities
The RN Case Manager builds relationships with area facilities and referral sources and provides skilled clinical care to hospice patients in their homes in the Northern Dodge County region.
Qualifications
- Valid Wisconsin driver’s license and proof of current car insurance are required upon hire and must be maintained throughout employment.
- Must be able to drive to and from care facilities, patient homes, and/or other locations as necessary for your role.
- Must be able to maintain healthy boundaries and positive interpersonal relationships.
- Must be tactful and courteous in communication with others, whether in-person, by phone, or in writing.
- Must act with integrity and demonstrate good work ethics including reliable attendance and punctuality.
