Location: Northern Dodge County
Position Type: Full Time
Schedule Requirements: 32-40 hours a week in Home Hospice. Position requires back-up, weekend, and holiday call rotation.
Sign-On Bonus: This position is BONUS Eligible after 6 months of successful employment. Bonus range is $3,500 - $7,500 based on hospice experience.

Purpose of the Position
Due to exponential growth and increased demand in Northern Dodge County, Rainbow Community Care is seeking a dedicated Licensed Practical Nurse to provide home hospice care to patients in their homes. The LPN provides practical care to patients under the direct supervision of an RN.
Key Responsibilities
- Appropriately demonstrates and teaches patient care techniques to the patient and/or family as directed by the RN.
- Integrates and demonstrates excellent standard infection control practice in all aspects of patient and family care and teaching opportunities.
- Uses the nursing process to assess patient care needs and applies to the plan of care.
- Demonstrates flexibility in work schedules and shared responsibility for delivery of care.
- Uses excellent organizational and communication skills to prioritize, supervise and manage daily work for self and other care team members.
- Able to use computerized charting and document within work shift.
- Possesses knowledge of specialized nursing skills for pain and symptom management and end-of-life care.
Professional Qualifications
- Current Wisconsin licensure as a Practical Nurse is required.
- Valid driver’s license and current auto insurance is required and must be maintained.
- Hospice experience preferred.
Personal Qualifications
- Ability to maintain patient and employee confidentiality.
- Ability to direct and apply problem-solving techniques in resolving issues.
- Drive to and from patients’ homes and to meetings at other locations as necessary.
- Ability to maintain healthy boundaries and maintain positive relationships.
- Strong written and verbal communication skills are required.
- Always presents self in a professional manner.
- Ability to act with integrity and demonstrate good work ethics, including reliable attendance and punctuality.
