Quality Life Services - Apollo is seeking a Discharge Planner/Social Worker who wants to make a difference in the lives of older adults. As a Discharge Planner/Social Worker you will be a pivotal part of the quality of life and care for our residents.
New grads welcome to apply We will train the right person with a bachelor's degree in Social Work or related field, Licensed Nurses encouraged to apply
- Coordinate with the interdisciplinary team to develop individualized care plans
- Participate in care plan meetings, resident assessment and MDS completion
- Assist with transitions to long term care and other levels of care
- Acts as a liaison between the resident, family and staff of the facility for any concerns
- Direct discharge planning by ensuring all discharge needs are in place prior to day of discharge and initiate discharge follow-up calls

Requirements
- Licensed in Social Work preferred but not required
- Experience in skilled care and/or long-term care facilities preferred
- Experience in the human services field
