Position Summary

We are seeking an experienced and motivated Registered Nurse (RN) to serve as our Regional RN Manager for the Concord, NH area. This is a unique hybrid role combining hands-on patient care with clinical leadership and team oversight.

The ideal candidate is a strong clinician, relationship builder, and growth-minded leader who thrives in a fast-paced, evolving environment.

Key Responsibilities

Clinical Care & Patient Services

• Provide skilled nursing visits for both home health and hospice patients
• Perform comprehensive assessments, develop and update individualized care plans
• Ensure high-quality, patient-centered care in compliance with regulatory standards
• Participate in on-call rotation as needed

Leadership & Supervision

• Serve as the lead RN for the Concord region
• Supervise, mentor, and support field clinicians (RNs, LPNs, HHAs)
• Assist with staffing coordination and clinical assignments
• Conduct supervisory visits and ensure clinician competency and performance

Growth & Operations

• Support expansion of services and referral relationships in the Concord market
• Collaborate with intake, scheduling, and leadership teams to optimize operations
• Ensure compliance with Medicare, CHAP, and state regulations
• Promote a culture of quality, accountability, and compassionate care

Qualifications

• Active RN license in New Hampshire
• Minimum 2 years of home health experience (required)
• Hospice experience strongly preferred
• OASIS experience required
• Prior leadership or supervisory experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities
• Valid driver’s license and reliable transportation

Pay: $40.00 - $55.00 per hour
