Position Summary
We are seeking an experienced and motivated Registered Nurse (RN) to serve as our Regional RN Manager for the Concord, NH area. This is a unique hybrid role combining hands-on patient care with clinical leadership and team oversight.
The ideal candidate is a strong clinician, relationship builder, and growth-minded leader who thrives in a fast-paced, evolving environment.
Key Responsibilities
Clinical Care & Patient Services
- Provide skilled nursing visits for both home health and hospice patients
- Perform comprehensive assessments, develop and update individualized care plans
- Ensure high-quality, patient-centered care in compliance with regulatory standards
- Participate in on-call rotation as needed
Leadership & Supervision
- Serve as the lead RN for the Concord region
- Supervise, mentor, and support field clinicians (RNs, LPNs, HHAs)
- Assist with staffing coordination and clinical assignments
- Conduct supervisory visits and ensure clinician competency and performance
Growth & Operations
- Support expansion of services and referral relationships in the Concord market
- Collaborate with intake, scheduling, and leadership teams to optimize operations
- Ensure compliance with Medicare, CHAP, and state regulations
- Promote a culture of quality, accountability, and compassionate care
Qualifications
- Active RN license in New Hampshire
- Minimum 2 years of home health experience (required)
- Hospice experience strongly preferred
- OASIS experience required
- Prior leadership or supervisory experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and organizational abilities
- Valid driver’s license and reliable transportation

Pay: $40.00 - $55.00 per hour
