We’re seeking a skilled Occupational Therapist (OT) to join our team, where you'll have the opportunity to make a meaningful impact on the lives of individuals and families in both hospice and home care settings.
Your Role and Responsibilities:
As an Occupational Therapist, you will:
- Assess Needs: Conduct comprehensive evaluations of patients' functional status and home environment, identifying equipment needs.
- Create Care Plans: Develop and communicate personalized treatment plans, coordinating with physicians and care teams.
- Enhance Daily Living: Teach and retrain patients in daily activities, adaptive equipment use, and prosthetics/orthotics.
- Support Hospice and Home Care: Provide specialized care tailored to both hospice and home care patients.
- Document and Report: Maintain accurate clinical records and report patient condition changes.
- Coordinate and Supervise: Ensure efficient care through communication with team members and supervision of therapy assistants.
- Educate and Train: Instruct patients, families, and caregivers on treatment regimens and therapeutic techniques.
- Participate in Care Planning: Contribute to team meetings and assist in discharge planning.
What We’re Looking For:
We’re seeking an Occupational Therapist who is not only experienced but also passionate about improving patients' quality of life. The ideal candidate will have:
- A degree from an accredited occupational therapy program.
- Registration with the National Board for Certification in Occupational Therapy (NBCOT).
- At least two years of clinical experience as an occupational therapist, with experience in both hospice and home care preferred.
- Strong verbal, written communication, and organizational skills.
- Current CPR certification.
- A valid driver’s license and a reliable, insured vehicle.
