Role: Provides support to Nutrition Services / Case Managers by assessing client needs, coordinating services, and maintaining records.
Key Responsibilities: Enters data into CIRTS, manages client files, communicates with clients via phone and mail, and collaborates with team members to establish services.
Prepares eligibility assessments, develops care plans, arranges services, and conducts follow-ups. Maintains mandated records, prepares reports, and correspondence.
Uses multiple software systems including word processing, spreadsheets, and CIRTS database.
Assists with obtaining holiday gifts and food for clients. Performs related duties as needed.
Qualifications & Skills: High school diploma or GED, at least 1 year of related experience, and required fingerprinting.
Possesses strong organizational, communication, and data management skills. Fully funded by grant, with responsibilities aligned to the grant's duration.
