Please note that this position is fully funded by grant funds. The expected duration of the grant funding is 3/31/2032. The position is funded for the duration of the grant and may be reduced or eliminated based on funding availability.
Enters data into CIRTS network; answers the telephone; mails requested information; and makes copies. Maintains client hard copy and electronic files. Uses several different software systems on different platforms; uses word processing, spreadsheets, and CIRTS database. Obtains holiday gifts and food for clients. Collaborates with others when needed when establishing services for clients. Performs other related duties as required. Completes eligibility assessment of clients; assists Case Managers in developing a care plan to meet the service needs of the client; arranges and coordinates needed services for clients; and provides follow-up with clients as needed. Maintains Federal, State, and County mandated client records; prepares reports; and writes letters and correspondence.
High school diploma or GED required. One (1) year of related experience. Fingerprinting required.

- Essential Employees may be required to work during an undeclared or declared emergency. The County’s Administrative Office will determine who will be required to work on an incident-by-incident basis.
