We are seeking a Clinical Care Liaison to service as a key liaison in coordinating patient transitions, referral management, and community outreach. This position is integral to our business development efforts and requires strong relationship-building skills with patients, families, physicians, and healthcare providers. You will educate stakeholders on end-of-life care, facilitate hospice admissions, and ensure a seamless transition for patients and their families.
Key Responsibilities
- Serve as the central point of contact for patient care transitions and business development.
- Conduct goals-of-care conversations with patients and families.
- Build and maintain relationships with referral sources, including hospitals, physicians, and long-term care facilities.
- Educate healthcare professionals and the community on hospice and palliative care.
- Conduct facility, home, and hospital visits to assess patient needs and facilitate referrals.
- Achieve targeted weekly referral and hospice admission goals per sales strategy.
- Participate in interdisciplinary team meetings and maintain accurate documentation.
- Complete required administrative tasks, including consents, medical forms, reports, and patient records.
Education
- Nursing Degree from an accredited college or university.
- Active and unencumbered Registered Nurse (RN) license in the state of South Carolina.
Experience
- Minimum 2 years of nursing experience in a hospital, long-term care facility, or related healthcare setting.
- Clinical and Sales experience in a hospice, healthcare, or similar setting is preferred.
