We are looking for full-time or part-time physician to join our growing team in Atlanta, Georgia. The physician will lead a team of Nurse Practitioners who are embedded in skilled nursing facilities through the region. The physician will be a key part of our structured, collaborative care model that is transforming the delivery of healthcare in post-acute care settings and that improves clinical outcomes and patient experience.

With a high-touch approach, TCPA helps ensure proper progress on plan of care and identify any early warning signs for readmissions during the critical, immediate post-discharge period.
Job Responsibilities
- Responsible for admissions and for executing the post discharge plan of care
- Responsible for ensuring safe discharge to home from the skilled nursing facilities
- Provide a true continuum of care to our patients alongside our facility partners
- Oversees nurse practitioners providing transitional care services in skilled nursing facilities
- On-Call is remote and on a rotation with other MD/DO within the company.

Qualifications:
Minimum Qualifications
- MD or DO
- Minimum of 3 years of experience working with geriatric population - preferred
- Board certified or board eligible in internal medicine, family practice or geriatric medicine
- Ability to lead and work in a team
- Excellent communication (written and verbal), analytical and problem-solving skills
- Entrepreneurial mindset with ambition to transform post-acute care delivery
