What are the responsibilities and job description for the Social Worker position at Providence Care?
Providence Care is hiring a Licensed Masters Level Social Worker to join our growing team to provide full time LMSW services to our hospice patients in their homes, assisted living facilities or skilled nursing facilities within the and surrounding counties.
The Hospice LMSW is responsible for the provision, direction, supervision and evaluation of medical social services to clients in the home environment in accordance with agency policies and procedures, applicable health standards, governmental laws and regulations. The Hospice MSW will strive to proactively meet the needs of the client in the achievement of individualized and measurable outcomes.
Responsibilities:
- Participates in development and re-evaluation of POC for clients needing MSW.
- Instructs and counsels the client and family in treating and coping with the social and emotional responses to illness.
- Participates in discharge planning.
- Participates in care coordination activities and act as resource to other team members in identification and resolution of client needs.
- Utilizes and connects patients and their families with community resources.
Qualifications:
- Masters in Social Work license in South Carolina
- Graduate degree in social work or related
- Excellent organizational, communication, and time management skills.
- Minimum of two years social work experience in a healthcare setting.
- Excellent interpersonal and analytical skills.
- Willingness to travel across several counties
