Key Responsibilities
- Provide comprehensive primary care, including evaluation, diagnosis, treatment, and chronic disease management.
- Perform physical examinations and review medical histories.
- Order and interpret appropriate diagnostic tests and develop individualized treatment plans.
- Manage medications, preventive care, and health-promotion needs.
- Collaborate with an interdisciplinary team to coordinate and monitor patient care.
- Educate patients and families regarding health maintenance, medical conditions, and treatment plans.
- Coordinate specialist referrals and follow-up care as needed.
- Supervise and collaborate with Nurse Practitioners and Physician Assistants.
- Maintain accurate and timely medical documentation.
- Participate in quality improvement initiatives and support compliance with healthcare regulations and organizational policies.
- MD or DO from an accredited medical school.
- Active California medical license required.
- Current DEA registration required.
- Board Certified in Internal Medicine or Family Medicine required.
- Minimum 1 year of experience working with frail or elderly populations.
- Geriatric care experience or additional qualifications in geriatrics required.
- Strong communication, clinical judgment, and teamwork skills.
