Responsibilities
- Evaluate and develop individualized treatment plans for geriatric patients
- Provide therapy focused on mobility, pain reduction, and functional independence
- Support post-surgical recovery, fall prevention, and long-term wellness goals
- Collaborate with PT team, aides, and administrative staff for coordinated care
- Document treatment in EMR according to Medicare/insurance guidelines
- Communicate effectively with physicians, caregivers, and family members
- Current New Jersey Physical Therapist License (required)
- Degree from an accredited PT program (DPT or MPT)
- CPR Certification
- Strong interpersonal and communication skills
- Experience with outpatient orthopedic and geriatric populations preferred
