What are the responsibilities and job description for the Senior Case Manager position at Project Hospitality?
SUMMARY:
The Senior Outreach Case Manager functions as the shift coordinator and ensures that the case managers
complete all tasks identified in the team meetings. Responsible for decisions related to team and client safety
while on the street.
RESPONSIBILITIES:
1. Fully responsible for making critical clinical decisions regarding clients while on the street.
2. Handling client crises as they arise.
3. Coordination of care in conjunction with other members of team.
4. Develop outreach approaches and develop treatment planning. Oversee information and referral work.
5. Outreach to community and city agencies and organizations to educate about PH outreach services.
6. Responsible to meet with Assistant Program Director each week to report on cases being worked on.
7. Manage data coordination and completion of reports in a timely manner.
QUALIFICATIONS/KNOWLEDGE NEEDED:
This position requires Bachelor’s degree with relevant related experience or a HSD with 3-5 years’ experience
working with the homeless, substance abuse, and mentally ill population. Excellent outreach and client
engagement skills a must. Must possess a clean and valid driver’s license and the ability to drive a passenger
van. Bilingual Spanish/English preferred. Must have excellent communication skills both orally and in writing
with individuals and groups in regard to complex and sensitive issues. The physical ability to perform the
essential job functions is needed. In addition, the Senior Outreach Case Manager shall perform any duties as
required to meet the needs of the program and the clients we serve. The agency reserves the right to review and
