Status: Full Time, Regular, Non-Exempt
Salary Range: $27.47 -$30.22 per hour [Approximately $50,000 - $55,000 annually]
Location: Long Island City
Days/Hours: Schedule varies: Monday through Friday, 9:00 AM – 5:00 PM and some
evenings, 12:00 PM – 8:00 PM. Subject to change depending on program needs.

The advocacy portion of our mission focuses on building a more just criminal legal system and reducing the barriers to reentry into society. The service side of Fortune has evolved to encompass a broad range of programs that started with employment and education and now includes a robust array of services, among them: alternatives to incarceration, supervised release, court advocacy, discharge planning within correctional facilities, licensed substance use and mental health treatment, a housing continuum ranging from emergency and transitional through permanent congregate and scattered site housing, assistance in obtaining benefits, a care management unit, HIV services, a food and nutrition program, and an arts program. Each service we provide is informed by the needs of the population we serve. Fortune currently serves approximately 14,000 people a year.. Our program models are recognized, both nationally and internationally, for their quality and innovation. We have locations in Brooklyn, The Bronx, Manhattan and Queens.
Position Summary:
The Senior Case Manager is responsible for case management duties, facilitating evidence-based groups, and overseeing case managers’ day-to-day responsibilities.

Essential Duties and Responsibilities:

Monitor and guide case managers in their responsibilities.

Perform chart audits of all case managers’ files in accordance with program protocol.

Ensure accurate and timely data entry of all information into the agency’s database.

Maintain and compile accurate data; submit monthly/quarterly reports to program supervisor.

Provide accurate and timely data and information in court reports.

Manage ATI toxicology processes, including labeling samples accurately and transferring toxicology reports and specimens to the appropriate destinations in compliance with program protocols.

Assist in facilitating groups using evidence-based curriculum.

Assist with program intake and assessments as needed.

Perform case management duties, including weekly individual sessions with clients, referrals to community-based services, developing client schedules, and treatment plans.

Assist clients in identifying and prioritizing needs.

Monitor client attendance and compliance with mandate requirements.

Conduct toxicology for clients as needed.

Develop and maintain optimum collaboration among departments/programs and external agencies.

Participate and conduct meetings and conferences.

Engage in ongoing professional development.

Qualifications:
Qualifications:
- Two or more years of recent case management experience.
- High school diploma or GED/HSE required; some college preferred.
- Commitment to working with mandated participants with criminal justice involvement.
- Bilingual Spanish a plus.
- Strong Microsoft Office and database entry skills.
- Strong assessment and engagement skills.
- Strong written and verbal communication skills.
- Strong organizational and leadership skills.
