What are the responsibilities and job description for the Senior Case Manager - Overnight - Outreach position at Project Hospitality?
Work Schedule: Monday through Friday 11pm to 7am
SUMMARY:
The Senior Outreach Case Manager functions as the shift coordinator and ensures that the case managers complete all tasks identified in the team meetings. Responsible for decisions related to team and client safety while in the field.
RESPONSIBILITIES:
- Fully responsible for making critical clinical decisions regarding clients while in the field
- Manage client crises as they arise
- Coordination of care in conjunction with other members of team
- Develop outreach approaches and develop treatment planning while overseeing information gathering and referral processes
- Outreach to community and city agencies/organizations to educate about Project Hospitality Outreach services
- Meet with Assistant Program Director on a weekly basis to report on cases being worked on
- Manage data coordination and completion of reports in a timely manner
QUALIFICATIONS/KNOWLEDGE NEEDED:
- Bachelor’s degree with relevant related experience or a High School Diploma/GED with 3-5 years’ experience working with the homeless, substance abuse and mentally ill population
- Excellent outreach and client engagement skills a must
- Must possess a clean and valid driver’s license and have the ability to drive a passenger van
- Bilingual Spanish/English preferred
- Must have excellent communication skills both orally and in writing
- with individuals and groups in regard to complex and sensitive issues
- Physical ability to perform the essential job functions is needed
In addition, the Senior Outreach Case Manager shall perform any duties as required to meet the needs of the program and the clients we serve.
Salary : $27 - $32
