Overview
We are seeking a dedicated and compassionate Case Manager to join our team. This role involves providing comprehensive support and guidance to individuals facing various challenges, including aging in the community. The ideal candidate will possess strong communication skills, experience in social work, and a passion for empowering clients to achieve stability and improved well-being. It involves the Community Care Program, the Adult Protective Service Program and Guide Program. Responsibilities
- Conduct thorough assessments of clients’ needs related to aging in the community.
- Develop individualized care plans that incorporate crisis management, motivational interviewing, and resource coordination.
- Coordinate services such as Medicare, meals on wheels, in home care aids, etc.
- Collaborate with community agencies, healthcare providers, and social service organizations to ensure comprehensive client support.
- Maintain detailed documentation of client interactions, progress notes, and service plans in compliance with organizational policies, ALL IN SOFTWARE!
- Assist clients with navigating complex systems including Medicaid and social security.
- Stop Abuse and secure a safety care plan as needed.
- Provide support specifically to Dementia and Alzheimer clients and care givers through the Guide Program.
Skills
- Strong background and bachelor's degree in social work or related fields such as health care administration, criminal justice, BSN/ RN
- Excellent communication skills with the ability to motivate and engage diverse populations through participant driven interviewing.
- Ability to handle sensitive situations with empathy, professionalism, and confidentiality.
- Familiarity with Medicare, Medicaid, and community resources is advantageous.
- Demonstrated ability to work independently as well as part of a multidisciplinary team. This position is integral to supporting individuals on their journey toward stability and self-sufficiency while fostering positive community relationships.
Pay: $38,000.00 - $41,000.00 per year
