Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating complex systems of care, ensuring they receive the necessary services and resources. This position requires strong interpersonal skills, a commitment to client advocacy, and the ability to work collaboratively with various stakeholders.
Duties
· Evaluate and process client applications and interview prospective clients for homeless prevention program and CSBG grant. May evaluate client applications for LIHEAP and weatherization.
· Data entry of client information.
· May organize and attend CSBG board meetings, state meetings, and supplementary training.
· Directs and coordinates activities concerned with implementation of objectives of program.
· Assist clients to assure successful implementation of homeless program.
· Review reports and records of activities to ensure progress is being accomplished toward specified program objectives. Modify or change methodology as required to redirect activities and attain objectives.
· Prepare program reports for superiors.
· Other duties as assigned.
Job requirements
· Apply problem-solving principles: define problems, collect data, establish facts, and draw valid conclusions
· Interpret technical instructions in mathematical or diagrammatic form.
· Ability to apply mathematical operations and calculations
· Ability to read a wide range of material, including manuals, books, technical and scientific literature.
Qualifications
Candidates should possess relevant experience in case management or related fields.
· High school diploma or GED required.
· Must have Valid Drivers License
· Must pass a background check.
Pay: $15.00 - $17.50 per hour
