Key Responsibilities
- Conduct Comprehensive Medication Reviews (CMRs) with eligible Medicare members to improve medication understanding and adherence
- Perform proactive outreach to Medicare members to address and remove barriers to medication adherence
- Collaborate with pharmacies and providers to resolve adherence gaps when members are unengaged or need additional support
- Identify and close SUPD (Statin Use in Persons with Diabetes) gaps through provider engagement
- Communicate with providers by fax or CMR platforms to recommend interventions (e.g., drug interactions, dose adjustments, adherence concerns)
- Provide medication counseling and education to Medicare members
- Educate members on pharmacy benefits, including formularies, preferred pharmacies, and supplemental benefits
- Support Pharmacy Quality Specialists with clinical guidance and expertise.
- Maintain accurate documentation and ensure compliance with quality and regulatory standards
Required:
- Bachelor’s degree in Pharmacy or advanced degree (PharmD preferred)
- Active North Carolina pharmacist license and residency
- Minimum 3 years of experience in a related field
- Strong knowledge of medication therapy management and adherence strategies
- Experience with Medicare, Medicaid, or managed care environments
- Familiarity with MTM/CMR programs and Medicare STARs metrics
- Background in retail pharmacy, health plans, or DSNP/HMO populations
- Strong patient communication and counseling skills
- Ability to manage high-volume outreach (phone-based engagement)
- Clinical knowledge of drug therapies, interactions, and disease states
- Detail-oriented with strong organizational and documentation skills
- Ability to work independently in a remote, fast-paced environment
- Background in retail pharmacy, health plans, or DSNP/HMO populations
