Job Summary
We’re looking for a Bi-lingual Spanish-speaking, Transition Care Specialist to join our growing team. In this role, you will support patients who are leaving care facilities and help them smoothly transition to our pharmacy service. You’ll serve as a point of contact, guide patients through the next steps, and ensure timely delivery of their medications.
Key Responsibilities
- Contact patients who are preparing to leave or have recently left a care facility
- Introduce yourself, explain our pharmacy service, and guide them through their next medication order
- Review patient profiles received from the facility to ensure accuracy
- Arrange and confirm delivery of the second fill (upcoming order)
- Keep patients informed and answer any questions regarding their medications or order
- Coordinate with pharmacy teams to ensure timely processing and delivery
- Manage caller queue tasks and assist with handoff to Personal Care Coordinators
- Communicate with other departments (pharmacy, billing, insurance) when needed
- Document all interactions accurately and follow up as necessary
- 1–2 years of experience in a customer service or patient-facing role
- High School Diploma
- Bi-lingual Spanish (reading/writing/speaking)
- Strong communication skills, especially over the phone
- Highly organized and detail oriented
- Comfortable using computer systems and managing multiple tasks
- Experience in healthcare or pharmacy is a plus
- Highly empathetic nature with strong communication skills
- Base Salary: Up to $40,000 (based on experience)
- Bonuses/Commissions: Performance-based; Total comp up to $50,000
